CMS suspends doctor-reimbursement cuts used to calculate ObamaCare

Democrats pushed the CBO to score their ObamaCare plan without considering a parallel effort called a “doctor fix” that would rescind already-scheduled cuts in Medicare reimbursements to providers. The CBO wound up scoring the bill both ways, thanks to a request by Rep. Paul Ryan (R-WI), and confirmed that the removal of the 21% cuts in reimbursements would make ObamaCare a deficit expander, running hundreds of billions of dollars in the red. Now we don’t have to wait for the Democrats in Congress to pay off the AMA for its support of ObamaCare, as the Centers for Medicare and Medicaid Services have temporarily suspended the cuts in anticipation of the “doc fix”:
Doctors slated to have their Medicare reimbursements cut 21 percent on April 1 got a reprieve from the Centers for Medicare and Medicaid Services, which is delaying lowered payments until after Congress reconvenes.
The agency, which administers the U.S. health program for people 65 years and older and the disabled, has ordered that claims submitted on or after April 1 be held for an added 10 days before being paid, said spokesman Peter Ashkenaz. That’s on top of the 14 days the Baltimore-based CMS usually holds claims. …
Unless Congress postpones or scraps the cuts, the average reimbursement for a colonoscopy would fall to $294.57, compared with the current national rate of $374.20, said Ellen Griffith, a CMS spokeswoman. For a typical visit to the doctor’s office, the reimbursement would fall to $51.70 from $65.67 for an established patient, she said.
The problem with the price-fixing scheme is that it’s narrowing the list of providers. Doctors can’t pay for their own costs now, and cutting reimbursements by 21% will only make the situation worse. It will force more providers out of the Medicare/Medicaid markets, the latter of which Congress expanded to include millions of new clients, which actually makes the situation worse for providers who want to remain part of the Medicaid system. The flood of new patients will displace those covered by private insurance, which will force them to go out of business or get out of Medicaid.
Of course, we knew that before Congress began to debate ObamaCare — and so did the Democrats. The AMA agreed to support their efforts only if Democrats rescinded those cuts. Instead of recalibrating the bill using honest accounting, they claimed deficit reductions that only exist without the doctor fix. For months, Democrats kept the rescission parallel to the overhaul of the health-care system to maintain the illusion of fiscal responsibility.
On April 1st, the illusion ends. When those cuts don’t go into effect, ObamaCare becomes a deficit-expanding program — and that’s assuming that the rest of it works as advertised, a very large assumption that flies in the face of experience from every government entitlement program ever created.

Medicaid is worse! My doctor only gets $19 for a Medicaid office visit. Obamacare expands Medicaid by covering up to 133% of poverty level. In KS thats almost 40% of the population. I can’t imagine our little clinic staying open when the majority of patients have Medicaid. The hospital won’t last long either since people with Medicaid pay ZERO when they go to the ER. I’m afraid it’s going to get ugly out here in the boonies.

The other day my doctor told me she makes less per hour now than her nurses. I did not believe her, now I think I do.
She likes Medicare patients though because Medicare pays quicker than HMOs.
It’s all about volume, 45 patients a day on average.
I note too that she, like my other doctor wonders why anyone would go into the field now.

This is good news in one sense. People, ie, docs, trust that the government isn’t about to really cut Medicare.
This is bad news in another sense. It’s like “cash for clunkers,” where only the biggest “fish” can afford to wait on the meal.
Implementation, just like with Bush, sucks.
I think the public gets the real story. Government is loaded with politicians, on both sides, who promise and don’t deliver.

I wish the GOP Gang of 41 would threaten to filibuster any bill containing the doc fix, as well as a stand alone bill. Their argument for doing so would be simple — OCare was sold to the American people with this cut in place. So here is what you really got.
Soon, docs would leave Medicare/Medicaid in droves and even longer lines/wait times would develop. The poor folk and seniors would quickly get irate, and calls for repeal of OCare would grow.
Sure, the GOP would get blamed for being cruel, cold, heartless bastids. But how is that any different than their normal treatment in the LSM now?

This cut is nothing new. Annual cuts to Medicare payments have been programmed for years. It is just part of the shell game played by congress, the cut is counted in deficit calculations then congress “delays” the cuts until the next year. This kicking the can down the road has been going on for sometime. The more rank dishonesty was to separate this from Obamacare to game the CBO numbers.

So between the 2 holds: 10 days and the regular CMS 14 day hold the doctors will have to wait 24 work days to be repaid. Now, Obama isn’t the only one who has expiration day itus. What if Congress doesn’t rescind the cuts? Does the AMA have any recourse? It was just a promise given to extract their support. The bill is passed. Does Nancy really need them anymore? Will the AMA and doctors be under the Obus? She has her 4 panels of the healthcare apocalypse – now the panels can just deem things to be cost effective or not.
Will doctors ask patients to come in Right Now so that they can get the paperwork in before the 1st?

Why Government Can’t Run a Business
The Obama administration is bent on becoming a major player in — if not taking over entirely — America’s health-care, automobile and banking industries. Before that happens, it might be a good idea to look at the government’s track record in running economic enterprises. It is terrible.
In 1913, for instance, thinking it was being overcharged by the steel companies for armor plate for warships, the federal government decided to build its own plant. It estimated that a plant with a 10,000-ton annual capacity could produce armor plate for only 70% of what the steel companies charged.
When the plant was finally finished, however — three years after World War I had ended — it was millions over budget and able to produce armor plate only at twice what the steel companies charged. It produced one batch and then shut down, never to reopen.
Or take Medicare. Other than the source of its premiums, Medicare is no different, economically, than a regular health-insurance company. But unlike, say, UnitedHealthcare, it is a bureaucracy-beclotted nightmare, riven with waste and fraud. Last year the Government Accountability Office estimated that no less than one-third of all Medicare disbursements for durable medical equipment, such as wheelchairs and hospital beds, were improper or fraudulent. Medicare was so lax in its oversight that it was approving orthopedic shoes for amputees.
These examples are not aberrations; they are typical of how governments run enterprises. There are a number of reasons why this is inherently so. Among them are:
1) Governments are run by politicians, not businessmen.
2) Politicians need headlines.
3) Governments use other people’s money.
4) Government does not tolerate competition.
5) Government enterprises are almost always monopolies and thus do not face competition at all.
6) Successful corporations are run by benevolent despots.
7) Government is regulated by government.

IMO, this is the part that needs highlighted, underlined and bolded:
CMS isn’t saying they will pay at the reduced rate, or at the current rate. They are just holding on to the claims for 24 days, instead of the usual 14, so Congress can decide what to do.
I hope healthcare systems across the nation are prepared to wait to find out if they get $1.00 or $0.79 on each reimbursable dollar, and that they have enough cash-on-hand to wait out Congress.
Oh, and if you’re looking for healthcare jobs? They’re still out there, but I’d bet HR/Admin are waiting for this to play out on any FTE approvals that can be delayed.

It would be a positive change to be paid on an average claim in a month. That is just the hold. Many claims are denied or sent back to the physician requiring rebilling.
The AMA was all for this, they did not “get” anything. The AMA is run by leftists. The doctors are being run over not the AMA.

One more point. If any hospital or physician was only receiving the current (before the cut) reimbursement across the board for all their patients it would be difficult if not impossible to survive. Better paying patients are already subsidizing Medicare patients.

Here in rural SW ND we have been facing the same problems.
A PA there through the week w/ a rotating Dr about once per week, if that.
Hospital A 80 miles from my house & it’s decent. Hospital B 15 miles from my house & it sucks.
Hospital B will most definitely close bcs of this.
And what Dr in their right mind wants to practice here?
Another reason we get Chinese & Indian Drs out here.
Then they leave.
And what will happen in the big city where I do 99% of my doctoring?
They’ll do better, but have to take on not only more patients bcs of this alone, but more patients bcs the rural hospitals & clinics will be closing.
Then a hundred mile one-way trip, or for many 200+ miles will be in order for just a clinic visit.
This is insanity.
