Smells like rationing spirit

posted at 2:21 pm on September 2, 2009 by Ed Morrissey

The supporters of ObamaCare insist that government control of health care will not lead to rationing.  However, a report late last week from Bloomberg gives a clearer indication of the kind of power ObamaCare will put into the hands of bureaucrats, and how they will control access to health care through the government checkbook.  Medicare has cut $1.4 billion in payments to cardiologists and oncologists in order to ration resources towards family-practice physicians (via Doug Ross):

While President Barack Obama and members of Congress have spent August debating health insurance and medical costs at public forums, specialists are waging what one advocate calls a “tooth and nail” fight against a separate initiative to boost the pay of family doctors, and cut fees for cardiologists and oncologists. The specialists, in newspaper columns and meetings with lawmakers, say patients will lose access to life-saving care, from pacemakers to chemotherapy.

The proposal by Medicare, the government insurer for the elderly and disabled, is an effort by Obama to focus U.S. medicine on preventive care. The fight by physicians who work with the most expensive patients is weakening support for Obama’s broader goal, legislation to remake the health system, said Mark B. McClellan, 46, a former Medicare chief. …

The cuts could have the unintended consequence of rationing care, especially in rural regions with a large number of Medicare patients, doctors said. In other areas, specialists may decide to pull out of Medicare, or ask patients to make up the difference with higher out-of-pocket payments, said Alfred Bove, president of the American College of Cardiology.

“A fair number of cardiologists are looking at the accounting and saying ‘we can’t afford it,’” Bove said in a telephone interview.

How will this affect costs for those who need oncological and cardio care?  It will force patients into hospitals rather than clinics:

Some oncologists in rural areas may stop offering chemotherapy in the office, forcing patients to travel to more- distant hospitals, said Allen S. Lichter, 63, CEO of the 27,000- member American Society of Clinical Oncology in Alexandria, Virginia.

That is exactly how rationing will take place in a government-run system.  Politicians will decide on priorities and adjust compensation to promote their engineering of the health-care sector.  Lower compensation will drive specialists out of the system, especially in disciplines not fortunate enough to have political support, just as happens in Oregon now.

What’s the result?  Fewer providers, higher costs, and more pressure on hospitals for treatment rather than clinics, where treatments are both more efficient and available.  We’re already seeing this in Medicare as providers stop taking new clients in the program.  The compensation supposedly controls costs, but it only controls pricing, and the costs of providing service outstrip the compensation.  Under those circumstances, providers have to either stop taking Medicare patients or fold.

From te January 31, 2008 NYT:

“Bush Seeks Surplus via Medicare Cuts

WASHINGTON — In his new budget, to be unveiled Monday, President Bush will call for large cuts in the growth of Medicare, far exceeding what he proposed last year, and he will again seek major savings in Medicaid, according to administration officials and budget documents.

……

Health care savings are a crucial part of Mr. Bush’s plan to put the nation on track to achieve a budget surplus by 2012. But before then, the officials said Wednesday, the White House anticipates higher deficits in 2008 and 2009, reflecting the current weakness of the economy and the cost of a stimulus package.”

So why the critism of Medicare cuts this year, and not last year?

Jimbo3 on September 2, 2009 at 2:26 PM

Obama — the very picture of the uninformed lay person — has this vague idea that if only there were better preventive care, then most expensive medical procedures can be avoided. His understanding of medicine is even worse than his understanding of history or economics, and yet the entire health care delivery system is supposed to bend to his stupidity.

Guess who will pay the price in denied care and higher taxes? Hint: It won’t be Obama’s family.

You miss the point, probably on purpose.

Most conservatives don’t have an issue with slowly reducing the MediCare program over time. The GOP is harping on it because it’s smart politics. The issue here is President Obama’s retard math that says we can cover everyone for LESS money without rationing. It’s impossible.

And “large cuts in the growth of Medicare” are not the same thing as cuts to the program. Are you being intentionally obtuse or are you really too dumb to know the difference?

And you’re completely missing the point. It’s that President Obama is trying to sell America on an impossible formula that we can cover more for less with no rationing. Health care is .. interesting.

It’s illegal to refuse to treat patients, so it’s effectively rationed only by whether the hospital can afford to stay open.

I’m not sure if it was better, back before WWII, when we were using ability to pay as the metric for rationing… but when hospitals close because people aren’t paying the bills….

What I’d like to see is the medical industry start getting more aggressive with deadbeats. Start to include pictures and fingerprints in patient records….

Since the Government is supported by us, for us, and at the mercy of us, logically at some point rationing would take place b/c no monies would be available as usual therefore, they, The Government, would be forced to raise taxes, something they say would not happen!

Taxes? How can a service be provided by the government, i.e. HusseinCare, and be offered for free?

As usual, an account would be setup for this hideous program called HusseinCare. As the account becomes top heavy, the account would be moved over into a general fund, just as they did with Social Security, and in the near future it would be broke again and require fixing! What, we did not learn from our past mistakes?

Simply moving SS into the GF was not the entire problem why SS is broke, Illegal Immigrants are burdening the system too! Illegal Immigrants will be covered under HusseinCare? Oh yes! Another burden that will also doom this hideous proposal.

OT/Just read that the American flag has been removed from the home page of The United States Mission to the United Nations. Unbelievable! But the Ugly Mug and one of his Ugly Thugs, Susan Rice, I presume, are front and center.

Death Panels simply mislabeled as rationing.

Thank You Baroke (Dr. Death) Obama!

Ogabe on September 2, 2009 at 3:26 PM

Seriously, if nationalizing healthcare is good for the people, how about nationalizing:

1. Automobile manufacturers (cars are too expensive!)
2. Drug manufacturers (drugs are too expensive!)
3. Farmers (food is too expensive!)
4. Clothing manufacturers (clothes are too expensive!)
5. Construction firms (houses are too expensive!)

This is fun! Look at all the free things I can get from the government! Why, I may never have to work again! Woo-hoo!

Andy in Agoura Hills on September 2, 2009 at 3:28 PM

And “large cuts in the growth of Medicare” are not the same thing as cuts to the program. Are you being intentionally obtuse or are you really too dumb to know the difference?

BadgerHawk on September 2, 2009 at 2:29 PM

Is that similar to what Ted Kennedy and cohorts decried “spending cuts” when it was a reduction in the increase.

belad on September 2, 2009 at 3:31 PM

No, but if you think what Barry says are lies, then Bush and Cheney lied about other things.

Jimbo3 on September 2, 2009 at 2:38 PM

Let me guess what you are alluding to:

“Bush Lied and People Died!”

“No war for oil!”

“terrorists have habeas corpus rights, detaining them without trial is unconstitutional”

Or this favorite lame lefty “moral equivalency” meme:

“Cheney and Bush are guilty of war crimes!”

Sounds to me like Jimbo3 needs to get some new lefty talking points from Herr Obama as his/her comment only proves his/her ignorance, proves he/she can’t think for themselves, and it makes him/her look childish by throwing out such comments when this thread is obviously about HCR, not Bush and Cheney!

Good thing there’s no rationing in our current system where for-profit bureaucrats make the decisions about who gets health care and who doesnt.

If Obama drops the public option then these lazy politicians need to kick their staffs in the rear, read the new legislation thoroughly and eliminate all of the socialist garbage that will undoubtedly remain. Obama is throwing a bone and they’d better not be too anxious to jump on it before the study it.

orange on September 2, 2009 at 3:34 PM

Hmmm….funny but if I remember correctly it is AGAINST THE LAW for a hospital to refuse treatment to anyone regardless of their ability to pay!

Now if your defenition of evil for-profit bureaucrats includes those in the hospital that Michelle is on the board of you could be on to something as I seem to recall she and the hospital board she sits on advocated patient dumping for those that couldn’t pay!

So, what was it you were saying about those evil for profit bureaucrats again?

Liberty or Death on September 2, 2009 at 3:42 PM

Lest we forget from Sarah Palin on August 7th:

As more Americans delve in the disturbing details of the nationalized health care plan that the current administration is rushing through Congress, our collective jaw is dropping, and we’re saying not just no, but hell no!

The Democrats promise that a government health care system will reduce the cost of health care, but as the economist Thomas Sowell has pointed out, government health care will not reduce the cost; it will simply REFUSE TO PAY THE COST. And who will the suffer the most when they RATION care? The sick, elderly, and the disabled, of course. The America I know and love is not one in which my parents or my baby with Down Syndrome will have to stand in front of Obama’s “death panel” so his bureaucrats can decide, based on a subjective judgment of their “level of productivity in society,” whether they are WORTHY of health care. SUCH A SYSTEM IS DOWNRIGHT EVIL.

Health care by definition involves life and death decisions. Human rights and human dignity must be at the center of any health care discussion.

Rep. Michele Bachmann highlighted the Orwellian thinking of the president’s health care advisor, Dr. Ezekiel Emanuel, the brother of the White House chief of staff, in a floor speech to the House of Representatives. I commend her for being a voice for the most precious members of our society, our children and our seniors.

We must step up and engage in the most crucial debate. Nationalizing our health care system is a POINT OF NO RETURN for government interference in the lives of its citizens. If we go down this path, there will be no turning back. Ronald Reagan once wrote, “Government programs, once launched, never disappear. Actually a government bureau is the nearest thing to eternal life we’ll ever see on earth.” Let’s stop and think and make our voices heard before it’s too late.

Sarah Palin

(BOLD PRINT FROM ME)

TRANSLATION:

At once Sarah Palin disabuses the notion put out by some Democrats that Obamacare will not lead to socialized medicine. Her indignation and opposition to Obamacare is clearly in evidence from the get-go.

She then debunks the premise of Obamacare reducing the costs of health care delivery and then affirms that the goal of government is to not live up to its moral responsibility but instead to ration care and refuse to pay the bill. That’s like a restaurant owner advertising he can lower the cost of a cup of coffee to its patrons but doesn’t tell them the reason he can do it is that he just cut all his staff’s wages in half.

Now the tie-in which is absolutely brilliant is when Sarah subtly implies that Obama does not represent ‘the America I know and love and that divergence from the American norm in terms of “death panels” and subjectively deeming who is worthy of care imputes evil intentions or machinations to a system that Obama and his bureaucrats have sanctioned.

Michele Bachmann has clearly been energized and emboldened by being mentioned in Sarah’s Facebook post. She is now talking about slitting wrists and taking a blood oath, calling Obamacare slavery (a Palin term) and being proud of being the second most hated female Republican woman.

Ezekiel Emmanuel’s credibility is now on life support and he is now referred to as Dr. Death. He has become the omnipresent symbol of doom, even more so than Obama, of what is wrong with Obamacare and hopefully soon its death knell.
Sarah also invokes Orwell and 1984 and Big Brother which proves she does read and knows what she is talking about: a great line to appeal to independents.

And finally Sarah pay homage to Reagan. She never forgets to throw some red meat to the base. Unless Sarah screws up big time the hardcore conservatives in America will never desert Sarah.

Liberty or Death on September 2, 2009 at 3:58 PM

A good friend, and retired air force officer just had his Medicare jump up 300%. He recently needed surgery or he would be dead in a year. Medicare told him he needed to wait three years. He wound up paying for the surgery himself. This is where the health care system is going, and Americans can sense it. Congress could care less since they have the best health care in the world, for life. They got to pick it and vote it in.
