Coming up: circumcision campaigns

The advocates of ObamaCare insist that medical decisions will remain between doctors and patients and not involve mandates from government.  However, the same people also cheer the idea of government “coaching” doctors to adopt practices, and to back up those choices with pressure from payment schedules, which will result in de facto diktats, especially when it evolves into a single-payer system.  If they succeed, expect to see a campaign to push new parents into circumcising their male children in the name of AIDS prevention — even though the risks are manageable and the effect less than certain:

Public health officials are considering promoting routine circumcision for all baby boys born in the United States to reduce the spread of H.I.V., the virus that causes AIDS.

The topic is a delicate one that has already generated controversy, even though a formal draft of the proposed recommendations, due out from the Centers for Disease Control and Prevention by the end of the year, has yet to be released.

Experts are also considering whether the surgery should be offered to adult heterosexual men whose sexual practices put them at high risk of infection. But they acknowledge that a circumcision drive in the United States would be unlikely to have a drastic impact: the procedure does not seem to protect those at greatest risk here, men who have sex with men. …

But Dr. Peter Kilmarx, chief of epidemiology for the division of H.I.V./AIDS prevention at the C.D.C., said that any step that could thwart the spread of H.I.V. must be given serious consideration.

“We have a significant H.I.V. epidemic in this country, and we really need to look carefully at any potential intervention that could be another tool in the toolbox we use to address the epidemic,” Dr. Kilmarx said. “What we’ve heard from our consultants is that there would be a benefit for infants from infant circumcision, and that the benefits outweigh the risks.”

Barack Obama has warned us of Tonsil Vultures and Foot Rustlers.  So far, he hasn’t issued a warning on Foreskin Fiends.  In 1999, approximately 65% of all males born in the US were circumcised, which represents around 1.3 million each year.  In contrast, surgeons perform only 600,000 tonsillectomies a year.

I’m neutral on the issue of circumcision, which has become a controversial practice, but find this idea of interventions very, very odd.  In the first place, circumcision does not provide an immunity to STDs, not AIDS or anything else.  Studies indicate that circumcised males may have less danger of acquiring an infection, but as the NYT points out, that’s from heterosexual relations — a very minor channel of AIDS communication in the US.  Men have much better choices than circumcision for avoiding HIV infection, including the use of condoms (still not a perfect defense, but better than circumcision), refraining from intravenous drug use with shared needles, avoiding high-risk sexual practices altogether, and so on.

Why should the CDC push circumcision at all?  The government has no business being in the middle of that decision.  Under ObamaCare, however, when the government starts paying more and more of the health-care tab, they will point to ambiguous cost savings down the road — in this and other cases, decades down the road — to pressure Americans into surrendering their choices now.

Update: Let me try to explain this in small words for people who like to give “awards” for hysteria but still demand to see Sarah Palin’s gynecological records.  If Obama says he’ll stop doctors from enriching themselves through unnecessary tonsillectomies and amputations, that is an explicit call for government control of health-care delivery and the interference of government between patients and doctors.  If the CDC — which is part of the same government that will control health care — decides that circumcision is beneficial and cost-efficient in the long term, that same mechanism would create pressure on doctors and patients to perform them.

You can’t have it both ways.  The government that has the power to stop Tonsil Vultures and Foot Rustlers also has the power to pressure for greater numbers of circumcisions.  The apparatus for that will be put in place, as Obama not only admits but brags.

This could be interesting. In my experience – nothing gets libs (esp. granola-type moms) all riled up more than a discussion about circumcision, usually they want to label those who are for it as barbaric masochists or worse. That Obama is mandating it without any choice by parents is really going to drive them crazy. Should be fun to watch.

Well I suppose this is a perfect example of the sort issue that comes up when you start pushing government run health care on people. Seriously with a procedure that is as controversial and personal as routine infant circumcision who would you rather make the decision? A well informed Parent in consultation with a trusted family physician or some government bureaucrat making an arbitrary decision based on the prevailing medical and political opinion of the day?

Speaking of blind, opening one’s eyes long enough to read the Times article and google around a bit reveals that this has not one damn thing to do with ObamaCare and has been an issue for at least a couple of years, and that the CDC folks in the thick (hah! they wish) of it are not appointees who are considering only “revising its guidelines…in favor of a more encouraging policy.”

That Obama is mandating it without any choice by parents is really going to drive them crazy. Should be fun to watch.

Except, you know, things like closing gay bath houses, requiring gays to be tested regularly, etc.

This push on circumcision is because it’s been somewhat effective in Africa. But the mode of AIDS transmission in Africa is heterosexual, unlike in the US. It has virtually no effect on homosexual transmission, and would do nothing towards stopping the disease here.

Those of you who know more about German history hopefully might disabuse me of that notion, but somehow I remember reading or hearing about it.

It wouldn’t surprise me, of course, given the Nazi takeover of the healthcare system being one of the first acts they took, which is a fact.

Some subcommittee staffer in communication with the CDC about regulations about an obscure corner of a bill that doesn’t exist inserting regs sure to make gun-totin’ compensating conservative males squirm? No.

A career (dedicated, intelligent) bureaucrat looking at a study that showed dramatic effects in Africa and deciding that even an incremental improvement of health statistics over here, based on that procedure would be good? Yes.

I’m snipped, my son is snipped and if I have more boys they’ll get snipped, too. There’s some slight advantages and minimal risks. Not only a lower risk of straight HIV infection, but other STD risks are slightly lower, too. Rates of childhood urinary infections are lower, and rates of penile cancer are lower.
Plus, it leave a man with a streamlined, modern looking tallywacker with 99% less smegma. Besides, 9 out of 10 prostitutes prefer a well-trimmed stiffy — are you going to argue against the experts?*

If it becomes mandatory through ObamaCare, though, I’ll make the kids jews for a day and do it the traditional way.

* Some statistics may have been generated by the Obama Administration Standard Methodolgy of Statistical Research (otherwise known as a complete rectal extraction). Use is at the user’s own risk.

The issue here isn’t about weather or not Circumcision would produce an incremental improvement of health statistics and lower rates of disease, it is however about choice and who gets to decide weather or not the procedure is performed at all, especially when it comes to infant children.

Personally I don’t have an issue with the procedure and can even agree with the logical benefit of circumcision based on the out come of the studies provided. However I vehemently disagree with the notion that the government has the right to mandate routine infant circumcision and in the process usurp the right of parents to make decisions about the welfare of their own children, based on the vague notion that government somehow knows best just because it is populated with dedicated, intelligent career bureaucrats.

TABoLK is right. As a former pediatric ER RN, circumcized boys have much, much lower rates of urinary tract infections than uncircumcized boys. Repeated catheterizations on a little guy are much more traumatic than a one time snip by a qualified doc. Current research shows lower rates of STDs in general, not just HIV.

Not sure why the government is involved but I speculate it’s another micromanaging, smoke and mirrors attempt to control the little people.

Adult males can be circumicized, but the thought of it makes me shudder. I want no part of it.

Medical practices tend to trend one way and swing back. The American Academy of Pediatrics has returned to promoting circumcision for health reasons.

It’s not so much that the procedure would be “forced.” It would be “encouraged.” As in, doctors will have to fill out a survey every quarter charting how many of their male patients were circumcised, how many times they counselled the parents about it, reasons why it wasn’t done, etc.

Most parents listen to their children’s doctors, and I already feel burned that so much “data” behind circumcisions in the past were either innacurate or completely fraudulent.

There’s nothing wrong with doing it if you’re completely informed, but I shudder when I see how many of them are done as mere routine procedures with little to no thought at all.

I just think we’re made the way we are for a reason, and I can’t imagine cutting off pieces of our children unless there’s a damn good reason.

Having a slightly reduced chance of catching HIV isn’t a good enough reason to mutilate a child any more than crazy-glueing a vagina can’t be justified by the reduction in unwanted pregnancy rates.

One of the side effects of a bad circumcision is tightness of the skin during erection so I wonder if Obamacare will pay for corrective plastic surgery or will it be an elective procedure.
