Obamateurism of the Day

posted at 8:05 am on August 13, 2009 by Ed Morrissey

Allahpundit posted this clip from Barack Obama’s town halls, adding feet stealers to the new ObamaCare lexicon of Evil Medical Professionals, a new entry after Tonsil Vultures. There is a point that the well-deserved avalanche of criticism misses, though.  The clip has Obama explaining that government needs to take over the American health-care system to stop doctors from getting rich off of amputations of limbs from diabetic patients:

On the doctors’ front, one of the things we can do is to reimburse doctors who are providing preventive care and not just the surgeon who provides care after somebody is sick. (Applause.) Nothing against surgeons. I want surgeons — I don’t want to be getting a bunch of letters from surgeons now. I’m not dissing surgeons here. (Laughter.)

All I’m saying is let’s take the example of something like diabetes, one of — a disease that’s skyrocketing, partly because of obesity, partly because it’s not treated as effectively as it could be. Right now if we paid a family — if a family care physician works with his or her patient to help them lose weight, modify diet, monitors whether they’re taking their medications in a timely fashion, they might get reimbursed a pittance.

But if that same diabetic ends up getting their foot amputated, that’s $30,000, $40,000, $50,000 — immediately the surgeon is reimbursed. Well, why not make sure that we’re also reimbursing the care that prevents the amputation, right? That will save us money. (Applause.)

But here’s what the Really Smart Guy in the Oval Office apparently doesn’t understand about the medical system.  Diabetics are treated by general practitioners and endocrinologists, neither of whom perform amputations.  The AMA pointed out that Obama was off by a factor of 100 on the costs, but thus far no one has pointed out that the incentive structure he assumes doesn’t exist.  Only after a GP and the endocrinologist, plus probably an orthopedist and a couple of other specialists, decide that the limb cannot be saved does the surgeon remove the limb.  It has nothing to do with the compensation that the GP and the endocrinologist receives, or an imaginary windfall for an amputation that will have nothing to do with either a GP or an endocrinologist.  I know this from long-time personal experience as the spouse of a Type 1 diabetic who lost her eyesight and kidneys to the disease, and who worried about amputations for years before her pancreas transplant.

If Barack Obama doesn’t understand that much about medical care in the US, why should we be listening to his ideas to radically restructure it?  Maybe he should quit shooting off his mouth, withdraw this bill, and learn something about the industry he expects to dismantle and reassemble.

Got an Obamateurism of the Day? If you see a foul-up by Barack Obama, e-mail it to me at obamaisms@edmorrissey.com with the quote and the link to the Obamateurism. I’ll post the best Obamateurisms on a daily basis, depending on how many I receive. Include a link to your blog, and I’ll give some link love as well. And unlike Slate, I promise to end the feature when Barack Obama leaves office.

Addendum: Normally I’d put a link to Chris Muir’s Day by Day site here, but today I want to do more than that. Chris has a fundraising drive to keep his excellent cartoon alive.  Be sure to drop a few dollars in his collection to keep one of the Rightosphere’s most talented warriors in the fight!

If Barack Obama doesn’t understand that much about medical care in the US, why should we be listening to his ideas to radically restructure it?

There is a great line near the end of the movie “The Devil’s Advocate” with Keanu Reeves and Al Pacino where as Reeves, as the son of Satan asks Pacino (Satan) “Why the Law Dad” and Satan replies “Because the law, my boy, puts us into everything” I think the same question could be asked of Obama, why health care? “Because health care, my boy, allows us control everything”

Ed, what he apparently does not know is that patient comnpliance is the key to treating diabetes. It is not the doctor whether surgeon or endocrinologist, but the patient who controls the outcomes of diabetes. But you will never see him telling his hordes of worshipers that they will be penalized if they don’t control their blood sugars because that is closer to the truth. Show me a diagnosed diabetic and I’ll show you a physician who has attempted to treat that person. The finger pointing ignorance with this man is amazing.

But these are not your doctors, they are a whole new growth industry that would arise out of the filthy liar’s plan. Essentially a government certified death squad to encourage you to do the right thing. Very Orwellian.

The fact that the government wants access to patients electronic medical records and bank accounts worries more than state-order exercise. Why do they need that data? Will you get a letter delivered by the (paragon of government efficiency) USPS notifying you that it seems you are not taking your diet seriously enough so they’ve deducted a fine from your account?

What the filthy liar is really trying to say is that preventive care is better than lopping off a foot. Fair enough as far as that goes but that isn’t sufficient to pay the bill for all the extras like providing preventive services for all the illegals.

If we really want to be fair, Barry should admit that PATIENT COMPLIANCE is the biggest predicter of diabetes outcome and that no one can make the patient compliant other than the patient himself. That is not to say that some people have a more difficult disease to manage, but overall, if he were honest he would state the true the reason people have a lot of complications is because of poor compliance, not because of poor medical care.

Absolutely. And I think people know this instinctively, which is why the protests have developed so quickly and with so much anger. In a way, I’m glad that Obama and the Dems decided to start this fight rather than Cap and Trade. People see C&T as something more remote. They may understand it will hurt the economy, but it’s more distant to them.

But people understand on a visceral level how health care intersects with privacy and freedom and thus are standing and making their voices heard. They’ve woken the Silent Majority.

It has nothing to do with the compensation that the GP and the endocrinologist receives, or an imaginary windfall for an amputation that will have nothing to do with either a GP or an endocrinologist.

As a Chicago politician involved with the unions, Obama’s probably figuring the surgeon’s kicking back some $$$ for every amputation patient the GP refers to him, since that’s the way the people he’s known operate.

To me, that is the thing that really scares me about govt. health care. Can’t you see Diabetes Czars knocking on doors and checking fridges and cabinets for chips and candy. And Exercise Czars saying “You vill exercise!”

freemo on August 13, 2009 at 8:19 AM

‘Smith!’ screamed the shrewish voice from the telescreen. ’6079 Smith W.! Yes, you! Bend lower, please! You can do better than that. You’re not trying. Lower, please! That’s better, comrade. Now stand at ease, the whole squad, and watch me.’

A sudden hot sweat had broken out all over Winston’s body. His face remained completely inscrutable. Never show dismay! Never show resentment! A single flicker of the eyes could give you away. He stood watching while the instructress raised her arms above her head and — one could not say gracefully, but with remarkable neatness and efficiency — bent over and tucked the first joint of her fingers under her toes.

‘There, comrades! That’s how I want to see you doing it. Watch me again. I’m thirty-nine and I’ve had four children. Now look.’ She bent over again. ‘You see my knees aren’t bent. You can all do it if you want to,’ she added as she straightened herself up. ‘Anyone under forty-five is perfectly capable of touching his toes. We don’t all have the privilege of fighting in the front line, but at least we can all keep fit. Remember our boys on the Malabar front! And the sailors in the Floating Fortresses! Just think what they have to put up with. Now try again. That’s better, comrade, that’s much better,’ she added encouragingly as Winston, with a violent lunge, succeeded in touching his toes with knees unbent, for the first time in several years.

If Barack Obama doesn’t understand that much about medical care in the US, why should we be listening to his ideas to radically restructure it?

The great tragedy is that he doesn’t understand anything except power politics and self promotion.

Let me apologize to all the type 1 diabetics. In my previous posts I was refering to Type 2 diabetics, not type 1 diabetics. Type 1 diabetes is much harder to control and will often lead to complications even with excellent patient compliance. Type 2 diabetes is totally controlled by lifestyle and/or medications compliance. Type 2 diabetes is also a huge cost to treat. I hope I did not offend anybody and if I did, I am sorry.

Under Obamacare, they won’t allow you to be sick of Obama. They’ll force indoctrination on you. (that was only slightly a joke)

We were waiting in the pediatrician’s office for my daughter’s physical for college and Obama came on the TV. My daughter put her arm up and said, “Do I have to listen to him?”

Beaglemom on August 13, 2009 at 8:50 AM

So, in Obama’s new world, what is the medical equivalent of FedEx and UPS?

I think doctors ought to show some collective muscle like SEIU members. How about a coordinated work slow-down–no elective surgery, botox, or well-visits perhaps. That ought to get some attn. I hope they don’t simply disappear quietly into the night.

JiangxiDad on August 13, 2009 at 8:55 AM

What the filthy liar is really trying to say is that preventive care is better than lopping off a foot.
highhopes on August 13, 2009 at 8:29 AM

Furthermore, he implies that few doctors, if any are currently committed to preventative care, that’s why he’s claiming that his new program will be able to save so much money.

‘Cause he and his Democrats are trying to cornhole America.

NoDonkey on August 13, 2009 at 8:55 AM

The reason Barack Obama thinks like this is because he operates in the same way. He’s out to short cut people out of money and resources, so he thinks the whole world is that way. Trusty prisoners in my local courthouses think the same way.

It’s his default to think that surgeons are ruthless enough to amputate patients’ limbs so they can make a buck.

His assessment says a lot about his ignorance of the health care industry, but it says a lot more about him.

He’s projecting again.

Mark, sorry to hear about your wife. I can sympathize. My wife’s diabetic too. There is hope. When my wife was diagnosed she dug in did her homework regulated her diet and lost the weight her doctor recommended. She now has it completely under control, has even discontinued medication. She still tests regularly and goes in for routine checks. Her doctor is amazed at her tenacity. Hang in there and be supportive, it’s not easy to achieve and it’s even harder to maintain but it can be done.

Oldnuke on August 13, 2009 at 8:56 AM

But you will never see him telling his hordes of worshipers that they will be penalized if they don’t control their blood sugars because that is closer to the truth

My Dad was a type-2 diabetic and lost three toes to it. He had a powdered sugar donut every morning for breakfast and ate the entirely wrong things for lunch (only dinner, when he wasn’t working, was anywhere near healthy, and that was because it was cooked by my Mom). But it was his doctor’s fault according to Giggles. Not the fault of the powdered sugar intake every morning.

Giggles really is a jackass, and dumb as a box of rocks too boot.

Or can I say “to boot” or will some doctor amputate my foot?

crazy_legs on August 13, 2009 at 8:57 AM

highhopes, bloggless

I have great respect for the Mayo Clinic. To be clear, I’m against nationalizing our health care! With that said, we do have a problem with medicaid and medicare cost since American taxpayers are footing the bill.

In my case, my GP, after obtaining an ultrasound (or whatever you call it), with the referral, sent me to the cardiologist. He immediately scheduled me for surgery and even mentioned cutting off my legs/feet. Long story, however this cardiologist will never see me again. I told him and his staff, as long as I’m paying, I still get to chose which Drs I use.

There is no doubt in my mind that Drs are “milking” medicaid and medicare and this should be fixed. The socialist dems should fix the problems and not change the entire system. As usual, the socialist dems go to far.

luckybogey on August 13, 2009 at 8:58 AM

Furthermore, even if an amputation does cost $50K, a lot of people have to get paid out of that money.

The surgeon, a surgical assistant, a surgical nurse or two, the anethesiologist. The post operative nurse.

Then there’s the cost of the surgical room. It has to be sterilized after each use. I’ve read that surgical equipment isn’t sterilized anymore, but discarded, so that a new surgical kit is needed for each operation. Those aren’t cheap.

The guys who run and maintain the hospital have to get paid.

And on, and on, and on …

MarkTheGreat on August 13, 2009 at 8:58 AM

How about Obama sticks to something he knows? Maybe he can stir up some racial resentment somewhere and get people out there to take “care” of us. I am so sick of him.
