Doctors oppose “public plan” in ObamaCare

The AMA has a long history of supporting government-subsidized health care, but has always drawn the line at a system of socialized medicine, ie, single-payer.  The current plans wending their way through the House and Senate would normally have gotten enthusiastic support from the AMA, but the association has balked at a key proposal from the administration:

As the health care debate heats up, the American Medical Association is letting Congress know that it will oppose creation of a government-sponsored insurance plan, which President Obama and many other Democrats see as an essential element of legislation to remake the health care system.

The opposition, which comes as Mr. Obama prepares to address the powerful doctors’ group on Monday in Chicago, could be a major hurdle for advocates of a public insurance plan. The A.M.A., with about 250,000 members, is America’s largest physician organization.

While committed to the goal of affordable health insurance for all, the association had said in a general statement of principles that health services should be “provided through private markets, as they are currently.” It is now reacting, for the first time, to specific legislative proposals being drafted by Congress.

The AMA wants the legislation to focus on leveraging private-market insurance instead of government-backed plans, and for good reason: they’ve dealt with Medicare.  Many doctors have given up on Medicare altogether, refusing to take any new Medicare-insured patients, because Medicare doesn’t cover the costs of providing their service.  Medicare is also a paperwork nightmare, as I can attest from first-hand experience.  They often withdraw payments already given to doctors a year or more later over some hiccup in their internal accounting.  The headaches of getting reimbursements makes Medicare business unprofitable.

However, as the New York Times’ Robert Pear reports, the public plan is the key element to ObamaCare.  The White House claims that they want to use it to keep insurers “honest,” but competition and sensible regulation do that.  They want to have an option to offer a loss-leader plan that will undercut the private insurers on price so that they will get forced out of the health-care sector, which will leave a single-payer plan in place by default.  The AMA understands this better than anyone, and will mobilize to keep from getting locked into a Medicare nightmare with no escape.

To a certain extent, though, the AMA brought this upon themselves.  Pear also reports that they broke with a trend of supporting Republicans in national elections in 2008, giving 56% of their donations to Democrats.  They got the Congress and the White House they wanted, even though Obama and the Democrats made it perfectly clear how they envisioned ObamaCare.  I’m glad to see the AMA fighting this, but they deserve the Captain Louis Renault Award for their shock, shock! that Democrats want to impose socialized medicine:

That might seem like a nice goal, but the reality is that it’s not going to happen. As other commenters have noted, if you start curtailing compensation for doctors, you’ll have fewer and less qualified doctors. You can’t force people to not be greedy. The best you can do is use their greed to achieve good results, which is what capitalism (along with certain regulation and incentive programs) does very well.

I think you’re right, mankai, re: SG’s age. No one who has actually been out and worked in the real world would be so clueless.

If you refuse to pay doctors, doctors will stop practicing medicine and go into some other line of business (this is already happening, btw), which will cause a constriction in the supply of health care providers…which, in turn, will lead to longer waiting periods which in some cases will prove fatal. And the doctors that are left will NOT be the “cream of the crop”, so to speak.

Basic economics. Why is it that Dems want to ignore that?

I work for a healthcare system. You have no idea what doctors really make, what their expenses are, how much insurance they have to carry for lawsuits, employees they have to pay, equipment they have to buy, continuing education they recieve to stay at the top of their game which is to provide excellent healthcare to their patients.

The AMA is pretty corrupt as well. The reason they don’t like this idea is because they call for an increase in doctors. The AMA artificially keeps the numbers of doctors low so that they can keep their rates high – and blame the rise in the COST of health care on insurance companies.

The administration calls for more doctors. That’s specifically what the AMA is designed to prevent. Of course they don’t like it. . . but they won’t tell you that is why.

We need more doctors whether we have Universal Coverage from the government or not. The AMA will not allow their sovereignty to be taken out by a mere president.

Yes, through the lowering of standards. That will work out great. What I want is a doctor who “doesn’t do well on standardized tests”, like Sotomeyer or Michelle Obama. So I can get a lawyer like Sotomeyer or Obama to sue them when they botch operation after operation. If I survive.

Good thing we have a seemingly endless supply of lawyers, however. We’re up to our armpits in them and somehow, they are all highly compensated it seems. Why is that, exactly?

This works in parenting, employment, job creation, innovation, creation…etc, etc. When you truly understand this you will understand how asinine you are being saying that medical professionals (who attend school for years and saddle themselves with hundreds of thousands of dollars worth of debt) should practice their profession without being compensated for it.

Anytime someone receives something for nothing there is the perception that it has no value. Just look at any government subsidized housing. The place can be pristine prior to having residents – then is reduced to trash status by the time the recipients of the governments largess move out. Check it out sometime, as it might open your eyes.

Here is what is needed: TORT REFORM – this is why the doctors order every test in the book if you come in with a headache. Afraid of trial lawyers and the insurance costs for doctors is outrageous.

Portable Insurance Across State Lines – would open up the insurance market for competition.

Walgreens, Target, Walmart and others are opening up clinics in their stores to see patients with flu, virus issues and other minor things that usually clog ER’s. The cost is about $50 – what the co-pay is for the ER. Makes sense and it’s the “free market”. Capitalism, remember that?

Cardiologists and other specialists make quite a bit more. Hospitalists are now the rage taking the pressure off the doctors. Pretty soon there will be hardly any doctors. These liberals in Washington hate this Country and want to control you. Wait til they can say “you eat at McDonalds? You can’t be treated”. Control, control, control.

I’m just suggesting that we have a cheat sheet – a Current Statist Troll List [CSTL] or some such as a quick reference.

It’s usually pretty obvious after just a couple of postings who these people are, and sometimes it would be useful if others consider them to be trolls with regards to whether and how to respond to them.

Pre-med college classes were hard. Med school was harder and just as long, and internship and residency were even tougher and longer. You don’t think so? Try it. Then run your own office, pay your staff, their health insurance payments, your malpractice premiums, supplies, taxes, and continuing education fees. Then complain about how much doctors make. Don’t forget the $100,000+ in student loans. My brother-in-law is a plumber and makes more than I do, without even a college degree. How long do you think the best and brightest will endure that kind of grueling training and debt if Obama caps their income at ever-lower levels?

It can be argued that excessive greed is evil or can lead to evil, but you seem to conflate greed with wanting to be compensated fairly for work which one has invested years and hundreds of thousands of dollars in to obtain the expertise required.

Grow up. Live in the real world and you’ll learn soon enough that it is not fair when someone steals from you to give to someone else.

Define it. You’re tossing around “greed” without a definition.

FYI, most doctors operate their own office, for which they have to pay rent. They hire their own assistants, nurses, and office personnel, whom they have to pay. They have to take Continuing Education courses to maintain their licenses (which can be quite expensive), and also have to pay a licensing fee to the state. Oh, and don’t forget malpractice insurance. One reason the number of OB/GYNs is declining so drastically is the ever-increasing malpractice premiums.

So. SouthernGal. What do you think a doctor should make? Let me tell you, $150,000/year may not cover all the expenses. So should a doctor go into debt (which they already have extensive amounts of due to the cost of medical school) to “serve” mankind?

I didn’t mean to ask when has ending greed ever worked. I was asking for examples where discouraging greed has led to good consequences. One example to the contrary is free market capitalism, which harnesses greed to achieve very good results. Examples from Europe that depart from the capitalism model do not discourage greed, they just don’t harness it in the same way. I don’t think it’s correct to say that Europeans doctors are less greedy than American ones. They just aren’t able to make as much money.

So, do you have any examples where greed is actually discouraged, and where this has led to good consequences? If not, it’s hard to really argue that discouraging greed is a good idea, especially when there are plenty of examples to the contrary.

Doctors pay for medical school. It’s even longer in school to specialize. I guess in your utopia that’s free too.

Doctors spend an amazing amount of time in unpaid training, working as interns in hospitals with insane, almost dangerously long shifts, getting abused regularly by tenured doctors…I guess they should tolerate this and not get compensated, just for humanity’s sake right?

Nobody else in the world works at their job or career for money, so doctors should be on call with hospitals and run practices for minimum wage in your utopia, right? Just because they want to help people get well, they shouldn’t expect to get decent compensation.

Well guess what toots….be prepared to go on a waiting list, not just for regular doctors willing to put up with your slavery like attitude of utopian ideals, get ready for the waiting list times 1,000 when you need a specialist.

THEN get ready for a pack of substandard, underpaid bumbling, overworked students attempting to diagnose you in a pack as they breeze by your hospital bed. Get ready for the misdiagnosis or no diagnosis or wrongly prescribed medication…if you get any at all. Get ready to be told that this illness, although suddenly having erupted, is all your fault because you didn’t practice preventative care, therefore aren’t eligible for the proper treatment. Just blow into this cardboard tube, you’ll be fine. Also, don’t plan on aging to age 50, because after that you’ll be 10 years beyond priority to even get on the 5 year waiting list to treat your illness.

Yay. Welcome to utopia.

And yes, you are greedy. So is everyone else. And they always will be. What the free market does is to harness the greed inherent in human nature, to benefit others. If you work hard and become a doctor and help people, you get well compensated.

In the system you are suggesting, the greedy people will be the people running the government. That is how what you’re suggesting, has always and will always work out. Because the government will be where the money is.

Is this for real?

Let me share a story (not for sympathy, just the truth). I just graduated medical school 2 weeks ago. Average debt is $182,000 from my school, at the current student loan interest rate of 6.8% over 30 year repayment (which is what most people have to do) that $180K turns into over $400,000. Then after medical school everyone has to do a residency (3 years at the shortest up to 7-8 years for things like neurosurgery). As a resident one makes between $45,000 to $50,000 a year working 80 hours a week. The Democrats are currently working really hard to get rid of student loan deferments during residency, because clearly on the aforementioned salary one can afford $1000 a month just on student loan payments (yes that’s the monthly payment even on a 30 year term). In my residency I will get an average of one weekend free a month and am scheduled to work July 4th, Thanksgiving Day and Christmas Eve this calendar year. Only after residency does one start making money, but you’re still working way over 40 hrs a week, and work weekends and holidays. Do I want people to feel sorry for me? No! I chose this life and despite the difficulties am ecstatic that I achieved my goal of becoming a physician. What does make me furious is the suggestion that wanting to be compensated for all the aforementioned hard work makes me greedy.

Ask them to produce a DD-214 and you’ll get a blank stare.

I worked for the VA, there are more phony “veterans” among the homeless than you can possibly count. We treated many of them for years, before someone actually had the gumption oto ask them to produce a certificate of discharge.
